In order for us to process your application in a timely manner you will need to fill out the Intake form by clicking the Download button below under Download Intake Form. Once downloaded, please fill out the information and email the completed form and the items 1-7 mentioned on page 1 of the Intake form to info@thrive2gethertherapy.com. Incomplete forms or missing items will cause delays in our ability to process your application.
